feat: Next.js + Tailwind + shadcn/ui project skeleton

- create-next-app with TypeScript, App Router, Tailwind CSS
- shadcn/ui initialized with defaults (button component + utils)
- .env.example with DATABASE_URL and SESSION_SECRET placeholders
- README.md with docs/runbooks/ reference
